Fraserview Retirement Community
Fraserview has been an integral part of the Richmond community for 45 years. The home was completely rebuilt in 2021 and provides care to 199 residents in 14 distinct neighbourhoods, each with its own dining, activity, and family areas. Every resident has their own spacious private room furnished with a single bed, wardrobe, chest of drawers, and nightstand. Fraserview is nestled in a quiet residential neighbourhood. The property boasts beautiful gardens with secure walkways, outdoor seating, and a gazebo.

What is the Richmond Long-Term Care Initiative?
The Family Practice Services Committee (FPSC) Long-Term Care Initiative (LTCI) supports Divisions of Family Practice to work with Health Authority partners to design and implement local solutions that deliver quality dedicated Family Physician Most Responsible Physician (FP MRP) in long-term care homes according to five best practice expectations (BPEs) and promotes three system level outcomes.
What are the LTCI’s five best practice expectations?
- 24/7 availability (supported by the LTC after hours call group) and on-site attendance, when required
- Proactive visits to patients – minimum every 3 months. Monthly visits are recommended.
- Meaningful medication reviews – upon resident admission and every 6 months thereafter.
- Completed documentation
- Attendance at case conferences – upon resident admission and annually thereafter.
What are the LTCI’s three system level goals?
- Reduced unnecessary or inappropriate hospital transfers
- Improved patient-provider experience
- Reduced cost per patient, as a result of a higher quality of care

What MRP Support is Provided through the Richmond LTCI?
The LTCI supports physicians throughout their LTC careers. When you join the LTCI, you will be provided with:
- LTCI orientation (includes meet & greet with the LTCI Physician Lead & care home tour)
- assistance with administrative onboarding (contracts, agreements, billing, etc)
- care home onboarding
- access to clinical mentorship (facilitated by on-site care home Medical Coordinator)
We also coordinate for you:
- access to the after hours LTC call coverage
- access to LTC tools & resources
- access to LTCI physician learning events, meetings, and mixers
- and more
What do I need to know about LTCI After-hours On-Call Availability?
Administration – LTCI physician after-hours on-call scheduling and funding is administered by the Richmond Division of Family Practice (RDFP). Call answering service used is the Personal Touch Telephone Answering Service.
Scheduling – The RDFP builds a rotating schedule every 6 months in accordance with LTC physician availabilities provided. Physicians on average cover 2 – 3 weeks total, annually.
Financial Incentive Eligibility – On-call availability payments ($2,500 per week)are in addition to billing, and are paid toall LTCI physicians participating in after hours on-callweekly shifts, regardless of their payment model. All FPs must participate in LTCI and the agreed upon call group structure in order to receive on-call availability payments from the RDFP.Call payment rates are based on prorated MOCAP level 2 rates for call availability.
Response Expectation – Calls must be answered within 30 minutes.
Simultaneous Coverage – A single physician may provide coverage simultaneously for more than one approved call group but can only receive payment on a daily basis for participating in one group. (Eg: If a FP is on-call for LTC and IPC, they may only be compensated for being on call for one program. If a FP is on-call for LTC and ER, they may only be compensated through the FPSC on-call availability funding or MOCAP).
What are the Richmond LTC Payment Model Options?
VCH LTC Clinical Service Contract – Receive pay monthly based on a maximum annual rate for providing a minimum of 0.2 FTE (one day per week) of services per year to a minimum of 25 residents at an LTC home, up to a maximum of 50 residents.
Longitudinal Family Physician Payment for Long-Term Care – The LFP Payment Model is designed as a comprehensive payment model for longitudinal family physicians, incorporating several aspects of family medicine care. It includes clinic-based services that are provided via longitudinal family physician clinics, pregnancy & newborn clinics, and patients’ home settings. It also includes facility-based care in long-term care facilities and palliative care facilities, as well as inpatient care and pregnancy & newborn care in hospital.
The LFP Payment Model is a blended payment model that compensates a physician for:
- physician time;
- interactions; and
- the size and complexity of a physician’s patient panel.
A physician bills for the total daily time they spend providing care, including time spent on direct care, indirect care and clinical administration. They also bill for each patient interaction and conferencing/communication. In addition, they receive a quarterly panel payment based on the number of patients on their panel and the complexity of those patients.

FPSC Long-Term Care Initiative (LTCI) MRP Payment – for physicians who are not eligible for the LFP Payment Model and provide care to LTC residents. Physicians receive direct payments from the Family Practice Services Committee (FPSC) for an amount of $290 per year per LTCI bed for which they are the Most Responsible Physician (MRP), to a maximum amount of $45,000.
To be eligible for this payment, you must meet all the following requirements:
- Not be enrolled in the LFP Payment Model
- Not be on a service contract, salary agreement, or sessional arrangement that includes payment for facility-based LTC services.
- Actively participate in the FPSC Long-Term Care Initiative (LTCI) with your local division, including the community solution for 24/7 availability and approach to MRP coverage for the beds in the community.
- Provide long-term care services according to the 5 best practice expectations (BPEs
- Be MRP for a minimum of 5 LTC facility-based beds.
- Meet all the requirements of the payment and commit to continuing to meet all requirements for the duration of the payment period.

Eligible family physicians will receive an invitation email at the beginning of each quarter from fp.billing@doctorsofbc.ca, which will include a link to an online application form and instructions on how to claim the payment.
After FPSC has reviewed and approved your application, Doctors of BC will send you an email confirming the payment. The funds will then be remitted to the same bank account that you use to receive sessional payments from Doctors of BC. The payment will be remitted six weeks after the claim form deadline.
