LTC physicians receive invitation and remuneration for participating in LTC physician meetings and LTC continued learning offered at minimum twice per year.
Role requirements - Practice in alignment with the Family Practice Services Committee 5 best practice expectations and 3 system-level outcomes
Resident Panel – Maintain a resident panel of at least 5 residents in designated LTCI beds and a maximum 50 residents per home (ideally 25) with a maximum of 100 residents per Physician. It is recommended that the Physician does not work across more than 2 care homes.
Proactive visits to residents - Attend proactive visits, commensurate with resident numbers and need. LTCI Physicians are required to visit their residents a minimum of once every three months and more frequently, if medically indicated. It is recommended that visits occur at minimum monthly, on set days (i.e., the first Thursday of each month).
Meaningful medication reviews - Complete medication reviews by on-site meetings or through the use of video or telephone alongside pharmacists and Care home staff, shortly after the resident’s admission and at least every six months thereafter and at any change in the resident’s status or after any transfer back from acute.
Completed documentation - EMR documentation. Provide completed resident documentation on the Facility care chart, including an admission history/physical identifying the resident’s current active problems and reasons for admission to long-term care, including completion of the MOST/DOI form, and documentation to the College standard of regular and urgent resident visits, care conferences and medication reviews. The LTCI Physician is responsible for ensuring completed documentation in the resident’s record after providing virtual care (through phone or videoconference) to residents.
Attendance at case conferences - Attend interdisciplinary care conferences on-site or through video or teleconference following the resident’s admission, and annually thereafter. Each LTCI physician attends one annual case conference per resident.
24/7 availability and on-site attendance, when required – Follow SBAR protocols, as well as communication workflows and guidelines provided by RDFP and Care homes. The LTCI Physician participates in an after hours calls group, which provides guidelines, for continuity of resident care during weekdays, holidays, and in the event of absences.
